Because of current browser limitations of the web, the membership application can not be made into an exact representation of the printed version. An exact representation, however, is available for download as an Adobe Acrobat document. (PDF file).

To see the membership application exactly as it was printed you will need a copy of the Adobe Acrobat Reader for Your operating system. A free copy is available from the Adobe website if you don't have it, just follow these simple steps:

1) Get Acrobat Reader

You can get a free copy of the reader from Adobe Systems by clicking on the "Get Acrobat Reader" button and following the instructions that you find there.

2) Install the reader

Once the reader has downloaded to your hard disk, you must install it by running the downloaded application.

• In Windows 3.x, use the file-run-browse menu and point it to the directory where the Acrobat Reader download was saved. Double click on the downloaded Acrobat Reader file, and click o.k.
• In Windows 95, select run from the start menu, and click on browse. Point it to the directory where the Acrobat Reader download was saved. Double click on the downloaded Acrobat Reader file, and click o.k.
• Mac Users may have to unstuff the downloaded file and install it with the Apple installer.
• The latest web browsers may include automatic installation procedures.

3) Get the membership application

Once you have the Acrobat Reader downloaded and installed, click on the membership application link above.
